# Compulsive Sexual Behavior

## What it is

Something that used to feel like a choice has stopped feeling like one. That is usually the sentence men say first, whether about pornography, about compulsive masturbation, about hookups or affairs, about hours lost to sexting or cam sites, about a specific fetish or platform that has quietly taken up more real estate than they meant to give it. The behavior itself is not the diagnostic marker. The gap between the behavior and what you actually value is.

The clinical name most therapists use is compulsive sexual behavior, or CSB. You may have also heard "sex addiction," "porn addiction," or "out-of-control sexual behavior." Those terms are related, and honestly, they are contested. The addiction framing has supporters and serious critics inside the field, and the diagnostic manuals do not uniformly endorse it. This page is not going to pretend that debate is settled. What we can say clearly is this: a lot of men experience sexual behavior that has become secretive, escalating, compulsive, or in direct conflict with their values, and that experience is real regardless of what label you pin on it.

Common signs the pattern has crossed a line, in plain terms: you've tried to stop or cut back and can't sustain it; the behavior is eating time, money, sleep, or attention you can't spare; it's happening in contexts you swore it wouldn't (work computers, around family, right before important events); the content is escalating in intensity, novelty, or risk; there's a growing secrecy layer with partners; and afterwards there's shame and self-recrimination that doesn't actually change the next week's behavior.

The Approach

## How therapy helps

The work begins with an honest, non-judgmental accounting. Not a confession. A map. When does the behavior happen, in what emotional weather, after what triggers, in what physical states (tired, drinking, alone, avoiding a difficult conversation)? Compulsive behavior almost always runs on a loop, and the loop has hooks you can actually see once you're looking for them. This piece alone is often clarifying enough that the shame layer drops several notches.

From there we work on three fronts in parallel. First, the behavioral piece: real structure around access, environment design, delay tactics, and if it's helpful, specific harm-reduction or abstinence protocols, chosen by you and matched to your actual life, not lifted from a workbook. Second, the underlying regulation piece: what is the behavior doing for you? Compulsive sexual behavior in men very often runs on unmet needs the man wouldn't name out loud, such as loneliness, anxiety, boredom, a stalled marriage, unresolved shame from decades earlier, chronic sleep debt. Those get addressed as real needs, not just as triggers to white-knuckle past.

Third is the values and identity piece, because behavior change that doesn't line up with who you want to be rarely holds. We're not going to write your values for you. We are going to help you say them clearly enough that they can actually steer the behavior. If your porn use is compatible with your values, we're not going to make you stop. If it isn't, and it's costing you your relationship, your attention, or your sense of yourself, the work is real and doable.

This is not 12-step, though we can integrate with a group if you're already in one. This is not moral coaching. It is direct, private, adult therapy for a pattern most men have never had a safe place to talk about honestly.

The behavior is a signal, not a verdict on who you are. Signals can be listened to and changed.
