## What it is

Erectile dysfunction is the persistent difficulty getting or keeping an erection firm enough for the kind of sex you want to have. It is almost never _only_ physical or _only_ psychological. It usually sits at the intersection. Blood flow, nerve function, hormones, medications, sleep, cardiovascular health, and mood all show up in the same equation, and untangling them is most of the work.

What makes ED so exhausting is the loop it creates. One or two disappointing nights plant the thought "what if it happens again," and that thought hijacks the next attempt. Performance pressure becomes its own cause. By the time most men reach out, the original trigger is long gone and the fear-of-failure cycle is running the show.

The Approach

## How therapy helps

Therapy for ED starts by mapping the actual terrain, which covers medical history, medications, how long this has been going on, what you've tried, and what a good outcome would actually look like for you. If a urology, cardiology, or endocrinology workup belongs in the plan, we say so and coordinate with your providers instead of pretending the physical side doesn't exist.

Alongside that, we work on the psychological piece using tools from sex therapy, including desensitizing performance pressure, breaking the spectator habit, rebuilding sensation- based intimacy with a partner, and rewriting the internal script that turns every attempt into a test. Education matters too: knowing how erections actually work at 25, 45, and 65 changes what "normal" means and takes a lot of the panic out.

This is territory I know from the inside. I founded FrankTalk.org in 2008 after my own prostate cancer recovery. It's now the largest online community for men with ED, and it informs how I work one-on-one. You don't have to translate. You don't have to explain why this matters. We just get to work.
