# Sexual Performance Anxiety

## What it is

Sexual performance anxiety is what happens when your brain steps offstage to watch, take notes, and grade the performance in real time. The internal monologue during sex starts running. "Am I hard enough? Is she into this? Am I taking too long? Am I about to finish too fast?" The more you monitor, the less your body cooperates, and the worry itself is the problem.

It usually starts with one bad experience. An erection didn't hold, an orgasm happened too fast, a partner said something that landed wrong. The next time, you show up _already worried it'll happen again_. That worry is a physiological event. It brings elevated adrenaline, tight muscles, and blood flow diverted away from where you need it, and it delivers the exact outcome you were afraid of. Now the loop is set.

The Approach

## How therapy helps

The work here is unusually concrete and unusually effective. Step one is naming the loop out loud so it stops running invisibly, then mapping the exact moments it fires for you. That means the shift from present to spectator, the specific fears that hijack the script, the avoidance patterns that grew up around it (fewer initiations, more alcohol, canceled plans).

From there we use sex-therapy tools with a long clinical track record: sensate focus to shift attention from performance to sensation, structured exposure to rebuild confidence in small increments, and cognitive work on the "if this doesn't work I'm broken" story that keeps the anxiety fed. If a partner is in the picture and willing, a session or two together defuses the mind-reading fast.

When there's a physical piece such as ED, PE, or medication side effects, we coordinate with your medical providers so the physical and psychological sides get treated as one problem instead of two. Performance anxiety is one of the most common and most treatable concerns in this practice.

Performance anxiety is not proof anything is wrong with you sexually. It's proof your nervous system is doing what nervous systems do, and that can be retrained.
