Most men wait years before talking to anyone about sex. Not because they are not bothered by it, but because the available options are a five-minute appointment with a physician who reaches for a prescription pad, or a general therapist who visibly hopes the topic moves on. Neither is built for this.

This practice is built for exactly this. Men's sexual health is the whole focus, the conversation is plain and adult, and you do not have to spend the first session explaining why it matters.

Who Paul is

Paul R. Nelson is a Licensed Mental Health Counselor (NY), a Licensed Professional Counselor (CT), and an AASECT Certified Sexuality Educator. His practice is men's sexual health: erectile difficulty, ejaculation timing, desire, performance anxiety, shame and body image, compulsive sexual behavior, recovery after prostate treatment, and the way any of those quietly reshape a relationship.

Because licensure is state-based, sessions are available to men located in New York and Connecticut. Both in-person and telehealth sessions are offered.

What working with him is like

Sessions run 50 minutes and they are conversations, not procedures. The first one is an assessment in plain language: what is happening, how long, in which situations, with or without a partner, on what medications, and what you want to be different. No questionnaires to score, no rating yourself on a scale, no forms to fill out before you have even spoken to anyone.

From there the work is specific. If the issue is behavioral, you get structured practice and a plan you will actually follow, adjusted when it stalls. If it looks medical, it goes to a physician or urologist and the therapy runs alongside that rather than pretending to substitute for it. If a partner is part of the picture, they can be brought in for a session or two.

Who this is for

Men who want to work on something specific and are willing to practice between sessions. Men whose erections or timing have changed and who want the psychological side addressed properly rather than medicated and forgotten. Men whose desire dropped off and who have quietly stopped initiating. Men carrying shame about their bodies or their sexual history. Men whose porn or sexual behavior no longer feels like a choice. Men rebuilding function and identity after prostate treatment.

You do not need a diagnosis to start, and you do not need to have it figured out before the first session. Sorting out what it is happens to be the first session's job.

Who this is not for

Couples therapy for a marriage in general trouble. Court-mandated evaluation, forensic assessment, or documentation for legal purposes. Prescribing, since that is a physician's role. Crisis care: if you are in immediate danger, call 911, or call or text 988 for the Suicide and Crisis Lifeline.

It is also not for men outside New York and Connecticut, which is a licensure limit rather than a preference. And if what you actually want is a promise that someone else will change, that promise will not be made here.

By the end of it you should have a clear read on whether the issue is behavioral, medical, relational, or a mix, and what the next step is.
